Placental vessels from type-1 diabetics and the effect of sodium nitroprusside

Background: Pre-eclampsia and placental insufficiency are associated with impaired trophoblastic invasion of the uterine spiral arteries and thereby with changes in the hemodynamic properties of the uteroplacental circuit. In previous studies we have observed a profound effect of the perivascular tissue (PVT) of the stem villous unit on the vascular tone. Furthermore we could relate this effect to the local release of nitric oxide (NO) from the PVT to the smooth muscle cells of the stem villous arteries (SVA). We hypothesize that the stem villous unit from pregnant women with T1DM have an altered response to NO compared to uncomplicated pregnancies.

Methods: The study setting is an experimental case-control study. Fresh delivered placentas from vaginal birth or caesarean section were obtained from cases (T1DM pregnancies) and controls (uncomplicated pregnancies) and the SVA dissected free under a stereomicroscope. The SVA preparations were mounted in a wire myograph (fig. 1) to record the isometric force development. Cumulative concentration-response curves to prostaglandin F2α (10^-8 – 10^-4 M) was made and the maximal contractility was used to assess the effect of a single dose (50 µM) of the exogenous NO-donor sodium nitroprusside (SNP).

Results: Our preliminary data show that SVAs obtained from pregnancies complicated by T1DM have an impaired dilatory response to a single dose of SNP. The difference in the dilatory response to SNP shows a borderline significance (p = 0.057) after 60 seconds and 120 seconds.

Conclusion: Stem villous arteries from diabetes type-1 placentas have an impaired response to SNP. Results are borderline significant.
