Abstract
Background: A recent study showed significantly higher risk of bacteremia among individuals with low socioeconomic status. No studies have focused on socioeconomic status as a risk factor for intensive care unit (ICU) admission with sepsis. We hypothesize that individuals with low socioeconomic status have a higher risk of ICU admission with sepsis than the background population.
Methods & Materials: This was a matched case-control study on patients with sepsis admitted to the ICU at Aarhus University Hospital, Noerrebrogade, Denmark, between November 2008 and October 2010. 387 sepsis patients were matched on sex, age and area of residence (Central Region Denmark) to 9-10 controls per patient (3,869) retrieved from the background population through Statistics Denmark. Socioeconomic status was defined as highest accomplished educational level, yearly income (based on yearly tax declaration) and cohabitant status, defined as either living alone or with a cohabitant. The odds ratio (OR) of being admitted with sepsis to the ICU was calculated using conditional logistic regression, adjusting for Charlson Comorbidity Index (CCI).
Results: The odds of being admitted to the ICU with sepsis after adjusting for CCI was significantly higher among the lowest income tertile (OR 2.17, 95 % confidence interval (CI) 1.56-3.02, p<0.001) compared to the highest income tertile. Compared to living with a cohabitant, individuals living alone had an OR of 2.63 (2.06-3.35, p<0.001) of being admitted. There was no significant association between educational level and the risk of admission to the ICU with sepsis.
Conclusion:
Individuals either living alone or having a low income had significantly higher odds of ICU admission with sepsis. The results indicate that this patient group needs specific attention when presenting with signs of sepsis.
