Expectations: What do we know and where do we need to go?

Abstract

Expectations are generally thought to be a key determinant of placebo effects. Yet, there is no consensus on how expectations should be defined, and placebo studies differ greatly on how—and if—they take measures of expectations into account. This chapter summarizes current approaches for the conceptualization and assessment of expectations, and presents new frameworks related to treatment expectations and prediction that may entail both conscious and unconscious aspects. These frameworks add to our understanding of expectations as complex, dynamic structures and may help to clarify when and how expectations contribute—not only to placebo effects, but also to treatment effects in general.
