Is the dynamics of glomerular capillary blood flow changed in diabetes?

In early diabetes, glomerular hyperfiltration may be found and is associated with a poor prognosis with respect to the development of diabetic kidney disease. By in-vivo 2-photon microscopy in diabetic rats, this project tests, if diabetic hyperfiltration is caused by changes in glomerular capillary blood flow dynamics due to dysfunction of glomerular mesangial cells. Mesangial cells are deeply involved in the later development of diabetic kidney disease by proliferation and production of extracellular matrix. The early appearance of hyperfiltration could indicate, that mesangial cells are affected also in early diabetes. New treatment strategies may focus on conservation and restoration of proper mesangial cell function in a preventive mode - at the onset of diabetes - before a reduced renal function or microalbuminuria is observed. Measurements of glomerular capillary blood flow dynamics in diabetic rats will allow studies on preventive therapies - potentially with known drugs like ACE-inhibitors - which can be translated to the clinical setting.
