Basophil Sensitivity Decreases During the Updosing of SCIT in Subjects Allergic to Grass Pollen

RATIONALE: Subcutaneous immunotherapy changes the immune response in allergic patients and results in a reduction of the specific type 1 allergic response. The basophil activation test reflects the allergic response and can be used to follow the development of the specific changes of the immune response. We hypothesized that changes in basophil sensitivity can be used to evaluate the humoral component of this reduced allergic response.

METHODS: We measured the changes in basophil sensitivity in 24 subjects with allergic rhino-conjunctivitis due to grass pollen. 18 participants received standard subcutaneous immunotherapy, another 6 were in the control group. Basophil activation was measured by flow cytometry as the percentage of CD63 expression on the surface of CD193+ blood basophils activated by 8 logarithmic dilutions of grass pollen extract (0,00001-100 SQU/ml) in cells reconstituted with present plasma, baseline plasma and washed cells. Half maximum activation (LC50) was considered a measure of basophil sensitivity and was used as the primary outcome measure.

RESULTS: In the cells reconstituted with present plasma the LC50 changed from -2.34 to -1.1 after reaching maintenance dose (p<0.001, n=18).This corresponds to an approximately 14-fold reduction in basophil sensitivity. No significant changes were observed in the baseline, washed cells or control group.

CONCLUSIONS: We show that the main mechanism leading to a reduction of basophil sensitivity is humoral.
