Beskrivelse
Every fourth person suffers from more than one concomitant long-term chronic disease (multimorbidity). A multisymptomatic and complicated disease pattern applies and, everyday-life of multimorbid patients is influenced. Multimorbid patients experience and are in reality exposed to lack of coordination and lack of interdisciplinary communication and care management. We meet these challenges by setting-up an innovative organization around multimorbid patients' trajectories by establishing Clinic for Multimorbidity in Denmark. Multimorbid patients are referred to the outpatient clinic by the patient's GP and undergo a consultation involing different specialities and health care professions. The aim of this study is to describe to Clinic and the multimorbid patients seen in it.
