Prediction of Post-Operative Sensory Disturbances by Panoramic Images and CBCT of the Mandibular Third Molar

AimTo assess whether canal-related parameters in panoramic images (PAN) and CBCT of mandibular third molars can predictpost-operative sensory disturbances.Material and Methods106 mandibular third molars examined with PAN and CBCT were removed. A temporary sensory disturbance of the inferioralveolar nerve (IAN) was present in 20 cases, and a permanent sensory disturbance in 1 case. Three oral radiologists(blinded to which patient had a disturbance) assessed PAN according to the relation between the roots and mandibularcanal: 1)Cranio-caudal relation (inferior/between borders/superior ); 2)Interruption of the canal borders (yes/no); 3)Positionof the canal over the roots (upper 1/3 / middle 1/3 / lower 1/3); 4)Darkening of the roots (yes/no); 5)Narrowing ofcanal lumen (yes/no); 6)Canal course (change/no change). The same parameters were assessed in CBCT in addition with:1)Bucco-lingual relation (canal between roots/same level/buccal/oral); 2)Canal in a root flex (yes/no); 3)Bony separation(no/yes); 4)Contact area (>½ root length/<½ root length/no contact). The first category in the brackets defined the “riskfactor” of each parameter. Positive (PPV) and negative (NPV) predictive values were calculated for all parameters.ResultsIn general, the NPVs were higher than PPVs for both modalities. The highest PPVs for PAN were found when the roots ofthe third molar were positioned inferior to the canal (0.31-0.33) and the highest PPV for CBCT was found when the canalwas positioned between the roots of the tooth (0.44-0.50).ConclusionCanal-related parameters observed in PAN or CBCT were not reliable predictors for sensory disturbances of the IAN.
