Pathological Findings in Panoramic Images as Indicators for Subsequent Findings In CBCT Concerning Maxillary Third Molars

AIM To assess the long term (>4-6 years) radiographic periimplant bone loss of implants installed in maxillary areas grafted with autogenous bone blocks, and use this measurement to classify these implants according to predefined success criteria. Material and Methods Fifty-seven patients had full maxillary alveolar ridge reconstructions using autogenous block grafts. In the augmented areas, 351 titanium implants were installed and followed-up after 6 months, 1.5, 3, 5, 8, and 10 years. Based on panoramic radiographs acquired at the follow-ups, the radiographic periimplant bone level was assessed, and the bone loss in relation to the baseline was calculated and used to classify the implants according to the predefined success/failure criteria presented by Albrektsson and coworkers/ALB (1986) and the Pisa Consensus Conference/PCC (2007). Results Mean radiographic periimplant bone loss was 0.53 mm during the first 6 months after implant installation, reaching 2.73 mm in the period >4-6 years. Different success criteria resulted in different prevalence of implants defined as failure. For the period >4-6 years after implant installation, approximately 50% of the implants would be classified as failure according to ALB, while according to the PCC, only 1% of the implants would be classified as failure, while 19% would be defined as compromised survival, and 45% as satisfactory survival. Conclusion Mean periimplant bone loss of implants installed in maxillary areas grafted with autogenous bone blocks was approximately 2.75 mm in the period >4-6 years of follow-up. Using different success criteria to define the prevalence of failing implants significantly altered implant outcome.
