The Optimal Dentition of the Periodontitis Patient: An Orthodontic Concept

This article focuses on the evolution of orthodontic treatment possibilities developed for degenerating dentitions in adult patients. Indications for such therapy comprise dentition suffering from an aggravation of a malocclusion, horizontal loss of periodontal tissue support, elongation and/or displacement of teeth as well as the need for a more favorable distribution and/or alignment of teeth in partially edentulous jaw segments. Orthodontics clearly play an increasing role in the treatment of patients whose dentition is degenerating, and the introduction of implants has paved the way for increased interdisciplinary collaboration between orthodontists and other specialties within dentistry. Interdisciplinary collaboration is undoubtedly the best way to provide such patients with a better prognosis in terms of occlusion, function and smile. In this specific context, various kinds of orthodontic implants, termed “Temporary Anchorage Devices – TADs”, have been designed to facilitate and enhance the effectiveness of orthodontic therapy. It became obvious that with TADs as anchorage, the displacement of teeth into edentulous atrophic areas leads to a build-up of bone that can serve to secure implants later on and thus facilitate the achievement of a maintainable, functional and esthetically satisfactory fixed prosthetic reconstruction of degenerated dentitions. Typical clinical examples of the different categories of degenerating dentition and their orthodontic treatment – underlining the preponderant role of dental implants – are presented in this article, including the importance of subsequent lifelong maintenance to guarantee stability of the achieved result.
